Thai Maternity Benefits Explained: Gold Card, Social Security, Civil Servant — What You Can Claim

Free benefits exist — but only if you know to ask. Thailand's maternity coverage reaches almost every Thai citizen, whether you work for a company, are self-employed, or are between jobs. What most families miss is knowing which system they're in and how to file the paperwork.
Every year, thousands of Thai mothers leave thousands of baht unclaimed — not because the coverage doesn't exist, but because they didn't know what they were entitled to. This guide maps out Thailand's four maternity insurance tracks, helps you identify which one applies to you, and walks through exactly how to use it.
References draw from the Social Security Office (SSO) [1] and the National Health Security Office (NHSO) [2]. For the most current benefit amounts — these can change with each fiscal year — always verify via hotline 1506 (SSO) or 1330 (NHSO).
Thailand's Four Maternity Tracks — Which One Is Yours?
Thailand runs four distinct health systems that cover childbirth. You can only use one primary track at a time:
| Track | Who qualifies | Managed by |
|---|---|---|
| Gold Card / บัตรทอง (NHSO) | Thai citizens not covered by any other system | National Health Security Office (สปสช.) |
| Social Security (Articles 33/39/40) | Employed, formerly employed, or self-employed contributors | Social Security Office (สปส.) |
| Civil Servant / CSMBS | Government employees and their registered family members | Comptroller General's Department |
| Private health insurance | Policy-holders | Private insurers |
Quick check — which track are you on?
- Work for a private company → Social Security Article 33 (employer deducts and remits for you)
- Left employment but continued voluntary contributions → Social Security Article 39
- Freelance, sole trader, or gig worker → Social Security Article 40 (if enrolled) or Gold Card
- Government employee or civil servant → Civil Servant track (CSMBS)
- None of the above → Gold Card (the universal default)
Gold Card (NHSO) — Free from the Start
The Gold Card (สิทธิบัตรทอง) is Thailand's universal coverage scheme, managed by the NHSO (สปสช.). It covers all Thai citizens who are not enrolled in any other system [2].
What it covers
- Antenatal care (ANC / ฝากครรภ์): Fully free at your registered hospital. Following กรมอนามัย guidelines [4], this includes blood and urine tests, ultrasound scans (as clinically indicated), and iron, folate, and vitamin D supplements.
- Delivery: All vaginal births and medically indicated caesarean sections are covered.
- Postnatal care: Six-week postnatal check, family planning counselling, and mental-health screening.
- Newborn care: Newborn screening and the full EPI vaccination schedule at no charge (see Thai Vaccination Schedule).
Key rules to know
- You must use your registered hospital (based on house registration / ทะเบียนบ้าน). Going out of network without a referral means paying out of pocket — except in emergencies.
- You can transfer your registered hospital by filing a request at an NHSO office or via the NHSO app.
- Epidural analgesia (การฉีดยาชาเข้าไขสันหลัง) availability varies by hospital — ask before your due date.
- Standard accommodation is in shared wards. Private rooms typically incur an out-of-pocket supplement.
Social Security — More Benefits Than Most People Realise
Enrolled workers have a separate and in some ways more generous set of maternity benefits than the Gold Card [1].
Eligibility
You must have contributed for at least 5 months within the 15 months before the birth month. Check your contribution history at sso.go.th or by calling 1506.
All four benefits
1. Antenatal care (ANC) reimbursement — up to 1,500 baht
- Covers prenatal visits whether at an SSO hospital or elsewhere; pay first, claim back.
- Keep every receipt.
- File using form สปส. 2-01 within one year of delivery.
2. Delivery lump sum — 15,000 baht per child
- Paid as a flat cash amount regardless of whether you deliver vaginally or by caesarean.
- Twins = 2 × 15,000 baht = 30,000 baht total.
- File using form สปส. 2-01 within one year of delivery.
3. Maternity leave wage replacement — 50% of average wage
- Calculated on your three-month average wage, up to a monthly ceiling of 15,000 baht.
- Total leave: 90 days. Typically the employer covers the first 45 days; SSO covers the next 45 days — confirm the split with your HR team, as company policies vary.
4. Child welfare allowance — 800 baht per child per month until age 6
- File using form สปส. 1-03/1 with the birth certificate (สูติบัตร).
- File early: back-payments are available but capped at 12 months.
- Both parents can be enrolled in Social Security, but the allowance can only be claimed once per child — decide between you who will file.
Civil Servant Benefits (CSMBS) — Most Comprehensive
The Civil Servant Medical Benefit Scheme is administered by the Comptroller General's Department [3] and provides the broadest coverage of the four tracks.
What it covers
- Antenatal care and delivery: direct billing to hospital — no upfront payment required for covered services.
- Coverage extends to registered spouse and children.
- Wide hospital network including most government hospitals.
What to do before pregnancy
- Register dependants (spouse, children) with your employing agency before you need the coverage — do not wait until delivery.
- Delivery at a private hospital outside the network may involve an out-of-pocket difference.
- Check your entitlement at cgd.go.th or through your agency's HR office.
All Four Systems Side by Side
| Benefit | Gold Card / NHSO | Social Security (Art. 33) | CSMBS Civil Servant | Private Insurance |
|---|---|---|---|---|
| Prenatal care cost | Free at registered hospital | Reimbursement up to 1,500 baht | Free (direct billing) | Policy-dependent |
| Delivery cost | Free at registered hospital | Flat 15,000 baht per child | Free (direct billing) | Policy-dependent |
| Maternity leave pay | N/A (not a labour-insurance system) | 50% of wage × 45 days (SSO portion) | Per civil-service leave rules | N/A |
| Child welfare allowance | None | 800 baht/month to age 6 | None (other benefits exist) | None |
| Hospital choice | Registered hospital (transferable) | SSO network | Broad government network | Very broad |
| NICU coverage | Included | Included | Included | Policy-dependent |
Important: You can only use one primary public track. You cannot stack Gold Card and Social Security. Private insurance, however, can layer on top of any public track.
Step-by-Step: Filing Your Social Security Maternity Claim
Before conception — get ready
- Log into sso.go.th and check your contribution months.
- Confirm you will reach 5 months of contributions before your expected delivery month.
- Choose your SSO-network delivery hospital.
During pregnancy — antenatal care
- Attend prenatal visits at an SSO hospital or pay elsewhere and claim back.
- Keep every receipt.
- File form สปส. 2-01 + receipts + copy of your national ID within one year of delivery.
Delivery day
Documents to bring:
- National ID cards (both parents)
- Social Security card
- Birth certificate (สูติบัต, issued by the hospital after birth)
- Hospital payment receipts (if delivering outside the SSO network)
- Form สปส. 2-01 (download at sso.go.th or pick up at an SSO office)
If you deliver at an in-network SSO hospital, the hospital handles the claim directly. If you deliver outside the network (e.g., emergency), you pay first, then file for the 15,000-baht reimbursement yourself.
After delivery — child welfare allowance
- File form สปส. 1-03/1 with the birth certificate and house registration (ทะเบียนบ้าน).
- The sooner you file, the sooner the monthly 800-baht payments begin.
Special Situations
Twin pregnancy
Social Security pays the delivery lump sum per child: twins = 30,000 baht, triplets = 45,000 baht. The child welfare allowance of 800 baht/month also applies per child.
High-risk pregnancy (ครรภ์เสี่ยงสูง)
Both Gold Card and Social Security cover specialist referrals. Your hospital issues the referral — no extra cost within the network.
Premature delivery and NICU
NICU (ห้องไอซียูทารกแรกเกิด) care is covered under both the Gold Card and Social Security. Transfer to a higher-level hospital may be arranged if needed.
Pregnancy loss — miscarriage and stillbirth
Both Social Security and the Gold Card cover medical care for miscarriage (แท้งบุตร) and stillbirth (ทารกเสียชีวิตในครรภ์). For Social Security: if the birth occurred after 28 weeks of gestation, some benefits may still apply — call 1506 for guidance on your specific situation.
Leaving employment during pregnancy
- If you contributed for 5+ months before resigning, you still qualify for the 15,000-baht delivery lump sum and 1,500-baht ANC reimbursement. You will not receive the maternity leave wage (no employer left to pay it).
- You can continue as an Article 39 voluntary member within 6 months of leaving employment to preserve future benefits.
- If you do not continue, transfer to the Gold Card by registering at an NHSO office or district office (ที่ว่าการอำเภอ).
Freelancers and the self-employed
Social Security Article 40 is available to the self-employed, but the specific benefit package differs — not all Article 40 plans include the full maternity benefit set. Check your plan tier at sso.go.th before relying on it. If you have not enrolled in Article 40, the Gold Card is your default.
Cross-border or mixed-nationality families
A Thai child born to a Thai mother holds Thai citizenship and qualifies for the Gold Card or whichever track the Thai parent carries. The Social Security child welfare allowance of 800 baht/month applies to children of SSO-enrolled parents regardless of the other parent's nationality.
Common Mistakes That Cost Families Money
- Not keeping ANC receipts — Social Security requires receipts to reimburse. No receipt, no reimbursement.
- Filing after the one-year window — both the ANC and delivery claims expire one year from delivery.
- Delaying the child welfare registration — back-payments exist but are capped at 12 months; file immediately after birth.
- Delivering out-of-network without knowing — you can still claim the 15,000 baht, but you must file yourself and have the documentation.
- Insufficient contribution months — check your status at sso.go.th before your due date, not after.

Summary: Know Your System, Claim What's Yours
The "Gold Card is for the poor, private is for the rich" framing misses the point: both Gold Card and Social Security are entitlements you have already paid for through taxes and contributions. Many Thai families — across all income levels — use the public system for delivery and add private coverage only for a private room or extra amenities. That is not settling for less; it is sound family financial planning.
Three things to do before delivery:
- Identify your track — Article 33, Gold Card, civil servant, or Article 39?
- Verify your eligibility — call 1506 or 1330, or check through the app.
- Prepare your documents — ANC receipts, national ID cards, birth certificate, SSO forms.
The entitlements are there. The work is knowing how to collect them.
